Mandatory Team Check-in Friday, May 28th from 6 - 10 p.m.
at the Four Points Sheraton.

  1. 2 copies of the Official Roster - signed stamped by appropriate registrar. Guest players to be added by hand.
  2. Player Pass Cards for all players, including guest players.
  3. Medical Release Forms for all players - Club forms or copy of signed USYSA registration form.
